Conservation Heritage license plates show how wild you are about fish, forest and wildlife conservation. For a donation to the Missouri Conservation Heritage Foundation, we will issue an Emblem Use Authorization (EUA) Statement, which allows you to order the Conservation Heritage plate of your choice.

We will direct your donation to Missouri’s nature centers, outdoor classrooms, stream teams, wildlife habitat and other conservation programs. Drive proud knowing you helped our next generation learn how to conserve, manage and use wildlife resources.

Step 1

To purchase Conservation Heritage license plates, first obtain an Emblem Use Authorization Form (EUA)* by making a $25 (1 year plates) or $50 (2 year plates) tax-deductible donation to the Missouri Conservation Heritage Foundation.**

4 Ways to Obtain an EUA:

1. ONLINE at MDC Permits. 

Click this link or the button below. Select the “Purchase your Permits and Applications” button. Then verify your personal information. When you reach the “Choose your Permit(s)” page with the permit/license options, you must select the “Plates” tab to see the license plate options. Select the 1 or 2 year option. Pay by credit card (MasterCard, Visa or Discover). Immediately print or save your EUA when finished as you will not have access to it once you leave the page.

2. By PHONE at 1.800.392.4115

Call during regular office hours to pay by credit card (MasterCard, Visa or Discover). There is a $1 convenience charge. You will receive a paper copy of your EUA in the mail.

3. By VENDOR

Visit a Department of Conservation hunting or fishing permit vendor, including Bass Pro, Conservation Nature Centers, and some Walmart stores. The vendor will print and give you the EUA upon purchase.

4. By MAIL Print this form.

Send completed form and check made out to MCHF to P.O. Box 366, Jefferson City, MO 65102. The EUA will be issued and mailed back to you.


In all cases, you must take a printed copy of your Emblem Use Authorization* (EUA) with you to the DOR office to purchase or renew your plates.


Step 2

AFTER YOU HAVE OBTAINED YOUR MCHF EUA Statement, fill out a specialty license plate application (Form 1716) here. Or you can contact the Missouri Department of Revenue at 573-751-4509 or visit one of their offices for the form.

When filling out Form 1716, choose your preferred style:

  • Stock – In step 2 on the form select the “Missouri Conservation Heritage Foundation (stock) ($15)” option in the middle column.
  • Personalized – In step 2 on the form select the “Missouri Conservation Heritage Foundation ($15)” option and fill out the information in step 3.

Three plate options

Conservation license plates are available in three designs (bluebird, deer or bass). Each animal tells a unique story about conservation challenges, environmental success and the responsibility of Missourians to ensure the continued presence of wildlife species in our state. The bass plate will go to support the Johnny Morris Institute of Fisheries, Wetlands and Aquatic Systems. 


Step 3

After filling out the specialty license plate application (Form 1716), you’ll need to complete the license plate purchasing process with the Department of Revenue. A $15 annual fee applies.

Mail the Missouri Department of Revenue (Department of Revenue Motor Vehicle Bureau, P.O. Box 569, Jefferson City, MO 65105-0569) the following items:

  • Your MCHF EUA
  • Specialty license plate application
  • Payment of $15

The Department of Revenue will notify you by mail when your plates are ready for pick up from your local license office. It can take 4-6 weeks to receive your plates.

Conservation Heritage license plates are available for passenger vehicles, motorcycles, and commercial vehicles that have a gross vehicle weight from 6,000 lb. to 24,000 lb.

The EUA will expire 180 days after the purchase date, so order your conservation plates soon after obtaining your authorization. You must purchase a separate authorization for each conservation plate.

Conservation plates may be ordered at any time, regardless of your current license plate expiration date, However, all Missouri specialty plates expire in July, therefore, we recommend first-time conservation plate holders purchase plates during the first half of the year.

Example of Plate Expiration:

If your regular license plates expire in February 2023 and you are purchasing a Conservation license plate for the first time:

  • One year registration –pay $25 (one year) and your license plates won’t expire until July 2024.
  • Two year registration – pay $50 (two years) and your license plates won’t expire until July 2025.
Replacement Plates

If you are needing to replace a damaged, or unreadable plate that has not yet expired, you may do so at any local license office, by contacting the Missouri Department of Revenue at 573-751-4509, or by going to the Missouri Department of Revenue’s website at https://dor.mo.gov/motorv/tabtheft.php and follow the directions given.

Fill out the 1576 (Application for Replacement Plates and Tabs) form along with covering the replacement fee. The fee is $8.50 per plate plus a $6.00 processing fee. If replacing both front and back plates the total would be $23.00. The form 1576 does need to be notarized and there is no additional charges for custom plates.

*The EUA is the receipt for your tax-deductible donation to the Missouri Conservation Heritage Foundation and is proof of donation that is required to order a plate from the Missouri Department of Revenue.

**Confirm your two-year eligibility with the Department of Revenue prior to making a donation; donations are non-refundable. If you purchase two year plates, save your returned authorization form to renew your annual tags the second year.
